A 49-year-old female with history of estrogen receptor (ER)/progesterone receptor
(PR) positive human epidermal growth factor receptor-2 (HER-2) negative breast cancer
gene-1 (BRCA-1) negative, multifocal invasive ductal carcinoma, presented with a 2
cm left breast mass and two of 26 positive lymph nodes. She is status post left mastectomy
with axillary lymph node dissection and immediate reconstruction with tissue expander
12 years ago. She also had risk reduction right mastectomy with a tissue expander,
and no cancer was found in the right breast. She then underwent adjuvant chemotherapy
with doxorubicin and cyclophosphamide, followed by paclitaxel. She received chest
wall and regional nodal radiation, completed 11 years ago. Patient was started on
tamoxifen, however rotated to toremifene due to tamoxifen intolerance.
